The operator was using the PPE Vending Machine but the machine got frozen and any of the doors were able to open The operator tried to reset the machine by turning it off and on to do this EE reach out to the power box on the top of the machine and when the operator moved the box a 2 5 metal dumb bell fell from the top of the machine striking the EE in the forehead Dumb bell was left on top of machine from past calibration of PPE machines from outside vender
