Mixing resin hardener sample air pucks

What happened

EE was opening a valve to the hardener to fill a bucket to mix it with epoxy This is a non standard practice normal operations this valve is not used The valve is designed to be used to check pump operation however at times operators use it to get small amounts of hardener The EE only needed a about a 2 cups of hardener The valve the EE was opening is pointed straight out toward the employee about 5 feet off the ground about chest level See attachment for pictures There are 2 other valves pumps in this system and they are both pointed toward the ground away from the operator There were 4 bars regular pressure but she lowered it to 2 bars When she opened the valve the hardening chemical shot out of the spigot with force and splashed her on the right side of her face chest arm hand and leg EE said she went to the bathroom to wash her face While there she noticed the other areas starting to burn She went to [REDACTED] and asked for help Supervisor was notified and EE was taken to the heal
