What happened
During the soap test process the dome from the vessel becomes dislodged and strikes EE on the right side of face and right arm This happened during 26inch trials the vessel passed the visual inspection there appeared to be no gap on weld Next step was performing soap test when filling tank with air the normal completed pressure for this is 10 psi this vessel the doom came off at 7psi The impact broke the operators safety glasses The EE stated could not see properly out her right eye EE was taken to the first aid room where first responders treated her First responder and Department Lead took the employee to the Hospital Production was halted until the investigation and corrective actions can be implemented
