What happened
Employee was helping load a Loose Load into a Trailer. When this is done someone needs to be on the top of the tier (Cartons) tied down to keep everything straight. [REDACTED] was on the tier and when it was finished (6 to 7 feet high) instead of letting someone help him down (which is the correct procedure) he decided to jump. After jumping he tried to preform a tuck and roll but failed and instead injuring his left shoulder.
