Before the incident
Moving steel drain cover plate back into place.
What happened
Employee was cleaning up spilled media and water from a broken pipe. As part of the cleanup he moved the steel cover plate off the floor drain so he could shovel all the spilled media and water into the drain. When the cleanup was finished he lifted the cover plate to slide it back into place but somehow he lost his grip and the steel plate fell onto his finger. This caused the plate to cut off the tip of his right ring finger.
