Employee taking the stairs down from floor two to first while talking on her cell phone. Employee tripped on last 2 steps and fell. Employee reported that she landed on her right ankle which rolled. Her right ankle hurt when she tried to walk. [REDACTED] needed medical attention as ankle was reported as swollen and she could not put any weight on it. Taken to U Health Tipton Hospital ER. Mrs. [REDACTED] never came back to work. She was informed that we were going to accommodate her restrictions but she has not answered any calls since. Technically she voluntarily resigned without notice. She also missed her [REDACTED] appointment.
