What happened
A lift driver was removing a full tote from the kit in [REDACTED] when it flipped over onto another team member. The tote hit the operator’s back causing a contusion. After inspecting the tote it was found that the bottom of the tote was broke missing some support footers that could have possibly prevented it from flipping off the forks. The team member was evaluated by the nurse immediately. The operator started to feel sore [REDACTED] and therefore was sent to the doctor for an evaluation. The lift driver was taken off the lift and recertified.
