What happened
At [REDACTED] [REDACTED] a forklift driver stepped off a forklift and asked another employee for help with emptying a box in the compactor. The box was on a pallet located on the forks of the forklift. After emptying the box the forklift driver started to pull the box back towards the forklift. While doing so they tripped on the pallet behind them fell onto the pallet and hit their head on the mast of the forklift.
