What happened
At the chop operation a shell failed to be chopped and the employee drug the shell down the conveyor then picked it up and placed it on a catwalk behind the chopper to reinsert it back into the conveyor line at a later time. After the employee sat the shell down and turned away the shell rolled off the catwalk and fell onto the employees right small toe.
