Before the incident
Employee was aligning wall girts in a boomlift.
What happened
The bolted connections are not slotted so the bolts must be removed to complete the girt alignment. The employee had secured the member with a chain-fall and removed the bolts and made the alignment. He then tack welded the member in place the tack welds broke causing the member to fall on one end. The member scraped his forehead and struck his right arm.
