Before the incident
Employee was walking along a structural steel I-beam connecting adjacent structural steel I-beams on the 2nd floor South side of the structure.

While tied off the employee lost his balance and reached for the guard rails of an adjacent aerial lift. While reaching for the aerial lift the employees [REDACTED] locked up and pulled him in the opposite direction. The employee then fell into his harness.
