Removing tooling from a forging press

The associate and his team leader noticed that the knockout pin underneath the die was getting stuck and not ejecting the parts from the die. The leader and an engineer decided to have the die pulled to investigate what the issue was. The team leader instructed the operator on the press to pull the die from the press. The associate failed to lock out the press before beginning his work. The press was fully energized and the knockout pin was pushing up against the bottom of the die with approximately 1500 psi. When the associate removed the final bolt in the die collar the die lifted out of the pocket crushing the associate 's left hand in between the die and the upper tooling. This severed the left index finger. Emergency medical was contacted [REDACTED]. They responded and took the associate out to Royal Oak [REDACTED] for surgery.
