What happened
Employee entered resident 's room to pass snack when she slid toward the sink falling to floor with left leg stretched out in front of her and right leg bent underneath. Employee attempted to catch herself with her right hand. Fall resulted in pain to legs back ankles and wrists. Employee reported there was a white lotion like substance in the floor.
