What happened
The suction cup lifting device was not in place properly because the adapter chain leading to the jib crane had too much slack in it prior to lifting the head. This caused the head to be off-set. Since it was off set the head landed on a small guide arm. He grabbed the head with his hand and slid it off the guide arm and dropped it onto his finger.
