He had feel asleep for several hours in his truck after his lunch period. He was awakened by his supervisor and came back in to work and was expanding coils.

Expander gun was not adjusted properly by the employee in which he was trained. Pressure escaped from gun and contacted his had that was misplaced on the gun.
