The employee was starting up the redline in the oil tempering department on the west side of the line. The employee noticed the wires starting to cross over and started to walk towards the crossed wires. The employee was so focused on getting to the wires and was not watching the ground in front of him as he was walking.

As the employee was walking towards the crossed wires the employee stepped on something metal causing his foot to slip out from underneath him. The employee landed on the ground and immediately knew something was wrong with his right ankle.

Broken right ankle.

The employee did not know the object they slipped on but the employee did state that the object was metal by the way it sounded going across the floor when he stepped on it. Witnesses from the area stated that they saw a metal chain laying on the ground right next to the injured employee. There was nothing else laying on the ground around the time of the incident and it was determined that the injured employee stepped on a metal chain.

Broken ankle from slip trip or fall caused by object or substance in walkway.
