Moving a beam off from forklift onto stack
What happened
The forklift operator was stacking some tubing beams on a pile. The beam was not sliding off properly so he moved the beams by hand. In the process he was placing a beam on another that had a tab sticking up and his fingers were caught between the beam and a tab on the lower beam.
Injury or illness
The tip of the index finger on the right hand was amputated and the tip of the middle finger was fractured and eventually also had to be amputated.
