Before the incident
IW was correctly standing on the floor directly in front of the slip rolls machine. He had been forming quite a few lollipop hangers which he has done thousands of times before.
What happened
IW reversed the slip rolls machine and then placed his hands behind the rolls. This is not necessary or the correct process. He got his gloved fingers too close and they were pulled into the rollers.

The pads of three finger tips were pulled off. The right middle and ring finger and the left ring finger.
