Before the incident
Removed a sub assembly radiator from the test tank and was placing it in the storage rack where it would stay until it is ready to be worked on.
What happened
When placing the radiator in the rack one of the poles used to secure the radiator was not fully placed into the support and the radiator tipped and the pole did not hold it in place. This caused the radiator to fall over striking the employee.
