Removing a valve block from a hydraulic cylinder.

Employee believed the pressure in the hydraulic cylinder was relieved and proceeded to remove the valve block. The pressure in the cylinder was not fully relieved and the valve block shot off and hit him.

Valve block struck the left side of his chest and neck causing lacerations. His head was also hit and fractured.
