What happened
A 16 piece of 2X4 was at the planer infeed when the operator reach down with his right hand and grabbed the piece of 2X4 he stepped on a knot which caused him to fall forward pushing the 2X4 into the planer. The 2X4 made contact with the still spinning chip head and pulled the 2X4 and his hand into the chip head.
