Driver was at stop in [REDACTED] at RCS 70 EDISON TECH LG Driver was on the dock and was attempting to turning the elctric jack toward the elevator doors Driver stubmled back pulled back on the power control which caused the electric jack to pin his left foot between and the pallet jack and the wall Pain level at 7810 had numbness coming back Pallet was loaded with frozen items Clean surface Had to offload 2 more pallets Pallet jack died and driver had to use manual jack for the other 25 This happened at [REDACTED] Driver did not call at the time of the injury because it was his last stop and didnt put him down so he was able to limp through the last 2 pallets Drove back from [REDACTED] with his last foot [REDACTED] was the start date training was done Only training completed was online video written test and observation observation was moving a few pallets
