[REDACTED] was forming outrigger on Break Press 11. After making the first bend it looks as if she accidently hit the pedal again when the part was not completely out. Resulting in 3 of her fingers getting pinched between machine and the part itself. Happened around [REDACTED] on [REDACTED]. Could not get the ram to go back up to release her fingers. Employees had to manually bend the part back to free them.
