On [REDACTED] EE had yearly hearing test showing right ear STS STS was confirmed on [REDACTED] audiogram Hearing questionnaires audiometric report and noise survey from 711 foundry and 545 DTO were emailed to audiologist for review He stated that hearing loss is work related and needs to be placed on OSHA 300 log Dr [REDACTED] wrote [REDACTED] had a pre existing loss in [REDACTED] when he was [REDACTED] The pattern was consistent with a NIHL Indeed his history is rich with a plethora of high noise sources firearms 170 dB snowmobiles 105 dB mowers 90 dB chainsaws 110 dB Even though his current exposure has been 72 2 dB TWA his previous work in the foundry presented an exposure of 93 1 dB TWA Based on this level of exposure it is plausible that his work exposure contributed to his change in hearing Therefore all the OSHA required FU action is needed
