Before the incident
At commercial delivery on top of Buck Hoist rifding up to 7th floor because the materials would not fit inside the hoist. Long metal studs layed on top of hoist and employee rode with the materials.
What happened
The Buck Hoist stopped before arriving to the floor. Employee looked out over the guard rail to yell at the hoist operator that he wasnt at the floor yet. Operator already knew and was continuing at a slower speed to the landing at the same time.
Injury or illness
Caught head between hoist 's perimeter guard rail and the floor 's landing plate. Multiple contusions and lacerations to head from hard hat 's headband and cuts and contusions to chin mouth nose and eyes.
