What happened
A third shift employee was running the centerless grinder adjusting the distance between the pusher wheel and the grinder wheel to accommodate the diameter of the boring bar. While the wheel was slowing to a stop the employee held the bar above the riser. The employee’s hand unintentionally lowered the bar into the grinding wheel. The rotating wheel forced the bar into the employees hand resulting in a laceration.
