Pulling a pallet to the back of the truck.
What happened
Did not have the pallet jack pumped up high enough and the pallet caught the edge of the threshold from the door to the back of the truck and shifted the pallet. He went to adjust the pallet straight on the pallet jack with his hand and when he pulled it his hand slipped and caught a small nail that was protruding from the pallet jack.
