Assembling machine

[REDACTED] bolted a retainer bar onto the machine and noticed that it was on incorrectly. He unbolted the bar and put 1 bolt in at the top which he did not tighten and the bolt fell out and hit [REDACTED] in the head.

Right side of the head (temple area) had a small area of blood and a knot was beginning to pop up.
