Operating the Bottle Uncaser on Line 6

The team member was working as an uncaser operator on [REDACTED]. They sustained a significant injury while on a walking working surface comprised of two elevated stationary work platforms. The two fixed stationary platforms were not securely fastened together and lacked a locking mechanism. The platforms became positioned in a manner that allowed sufficient space for the employees left leg to pass through the walking working surface resulting in the accident. During the fabrication or installation process of he stationary work platform with pultruded fiberglass grating the grating was cut leaving a sharp edge. This sharp edge is suspected to have worsened the severity of the injury.
