What happened
Worker was removing upright portions of the scaffolding to replace them with the correct size. He was wearing a full body harness with a double SRL. He forgot to connect to a new point and disconnect from the piece he was removing. He threw the section to the ground and it pulled him off with it. He fell about 12 feet.
