What happened
Setup was getting mixed material on [REDACTED] and needed to clean the filter on the hopper. He took off the top part of the hopper and went to set it a side and believed it was stable . He then set his hands on the rail and conferred with a colleague and the lid fell and pinched and opened the skin.
