Before the incident
Employee was performing QA tasks of two tools in the cleanroom walking on a tool pedestal.
What happened
Employee was walking between the two tools he was working on and during the relocation he was walking in between another set of tools not managed by Hitachi. One of the covers on the tool pedestal that the employee was walking on slipped into the pedestal hole losing footing and caused the employee roll his right ankle. The hard barricade was available but this tile was outside of the secured zone.
