He was working on oil heaters and molds for press 9 in which the top part of the mold wasn 't heating up. He made up a new line and had to change the fitting on the manifold for it to fit.

The other fittings didn 't want to come off so he tried removing the plug. He tried to break the plug free with a wrench. When he pulled towards himself the wrench came around and struck him in the chest.
