Before the incident

[REDACTED] was reviewing paperwork on top of drum at end of sampling aisle partially in traffic area. [REDACTED] was backing up out of storage area driving around some drums in aisle that were partially blocking his view.

What happened

TM2 drove around drums in aisle while backing down traffic area. TM1 was focused on & reviewing paperwork with while standing one edge of aisle and traffic area. TM2 backed fork truck on to TM1 's left foot.

Injury or illness

There was no visible injury or swelling but TM1 informed manager that he had discomfort in left foot. TM2 notified Manager of incident and grabbed ice packs. TM1 sat in locker room removed his shoe sock and placed ice on top & underneath is left foot.

Object or substance involved

The fork truck 's back right tire drove onto TM1 's foot and TM1 yelled. TM2 drove back off of foot. TM1 was wearing steel-toe boots but tire partially sat on the middle right top of TMs foot.
