Before the incident
Cleaning the production facility with a high-pressure hose (100 psi) using an acid based soap.
What happened
The nozzle clamp became loose on the 100 psi hose filled with an acid-based soap. The nozzle then fell off the hose. The hose smacked the employee on the left side of the head near the left eye. Left eye was immediately irrigated at the eyewash station for 10 minutes then an additional 5 minutes before transport to the emergency room. Employee was not wearing the protective eyewear.
