What happened
There was a helper operator to the right of the forklift and the affected operator to the to the left of the forklift. The operator on the right told the driver to move the bale forward which they did. The affected operator had finished tying the bale wire to the forklift backrest. They had their foot in front of the left drive wheel and was run over.
