[REDACTED] left a forklift unattended and on gear. The forks were left up and pinned against a cylinder but when [REDACTED] moved the cylinder with a crane the forks that were blocked by the cylinder caused the forklift to drive forward. [REDACTED] was not able to move out of the way in time which caused his foot to be crushed by the forklift.
