Loading a 12ft aluminum street light arm

[REDACTED] -[REDACTED] [REDACTED] had completed the service on [REDACTED] Utility pickup #4213. He tried to reinstall a 12 foot aluminum street light arm into the bed of the truck alone. He lost his balance resulting in [REDACTED] twisting his left ankle struck his left knee on the trailer hitch and his middle finger on his left hand caught between light base and light arm. [REDACTED] and [REDACTED] unloaded that same 12 foot aluminum street light arm on [REDACTED] because it over hung the pickup approximately 3 foot and would not allow the bay door to close for the weekend. He is being treated at the Wired for Wellness Center for his injuries.
