Service Point Inspection

On [REDACTED] at approximately [REDACTED] at [REDACTED] [REDACTED] [REDACTED] walked around his work van stepping onto the edge of the asphalt driveway and grassy area rolling his foot. This caused [REDACTED] to fall landing on his right shoulder. [REDACTED] immediately contacted his supervisor [REDACTED] to inform him of the incident as well as experiencing pain in his right shoulder due to the fall. [REDACTED] did not feel medical attention was necessary. However during the weekend [REDACTED] contacted [REDACTED] to inform him that his shoulder was still sore and he has set up an appointment to see a doctor on [REDACTED]. Upon informing [REDACTED] of the incident [REDACTED] requested for [REDACTED] to return to the office and complete an incident report.
