Installing metal plates around a conveyor belt.

The employee was standing on a platform waiting to unload mateiral from a pallet being lifted up to his level with a forklift. The forklift tips became stuck on the gaurdrail of the platform the employee was standing on. When the fork tips became unjammed they bounced forward striking the employee in the shoulder dislocating it.
