In the bathroom assisting patient to toilet and trying to get his right amputee leg out of toilet. Weight 201.2 kg height 5 '11. [REDACTED] - Adjusting patient to reposition towel behind patient. Nursing home placed towel on left shoulder for cushion on wheelchair. The towel fell down and had to lean patient forward to get towel. Arm was already sore from [REDACTED] incident and became worse after this incident. Weight: 163.5 kg height 6 '
