Supine to sit and sit to stand.
What happened
Patient was moderate to maximum assist to sit at the [REDACTED]. Due to pt 's overall weakness and posterior trunk lean writer has to provide heavy assist to maintain sitting at the [REDACTED] to prevent sliding out. Called out for help and RN assisted with further mobility. Used safe sit to stand DME instead of the walker. PT unable to utilize DME properly due to poor direction following and was excessively relying on writer and RN. due to excessive bending and twisting writer experienced constant deep ache in lower back. Which resulted in inability to stand upright.
