Before the incident
I was completing an initial therapy evaluation with a patient normally independent with mobility using no device. He demonstrated good strength and sitting balance. However upon standing he had a heavy posterior lean and noted to have a very ataxic gait with taking steps and started falling backwards upon backing up to toilet thus requiring increased assist to prevent him from falling onto the floor.
