What happened
A patient on the unit was noted to be presenting with labile mood tearfulness and agitation on the phone in the day room. Another patient approached the patient out of concern. Spontaneously he began attacking her physically with punches. [REDACTED] came out of the nurses ' station to intervene and when she did the initial patient began assaulting RN. This writer personally witnessed at least 6 punches to RN 's upper body. 631 staff brought patient to the quiet room applied physical restraints and gave the patient IM haldol.
