Training new associate on assembling the [REDACTED].

What happened

[REDACTED] [REDACTED] and New [REDACTED] were putting the nugget pellet mill back together after it had been cleaned. [REDACTED] [REDACTED] was training [REDACTED] on how to reassemble and set the proper distance between the rollers and the die. They had put it together but had to disassemble it multiple times due to an issue with the bearings in one of the rollers. The left side adjustments were successfully completed and they moved to the right-side adjustment. After [REDACTED] had loosened the bolts with a wrench [REDACTED] [REDACTED] came over leaned down to look into the mill. He rested his right hand on the door of the equipment and his left hand on the inside of the die for balance. At this time [REDACTED] rotated the die thinking that they had moved onto the next step and caught [REDACTED] [REDACTED]s left index finger between the roller and the die. This resulted in a crushing injury and creating an open wound in the inside crease of the first joint of his left index finger.
