What happened
The Wet Brush Operator proceeded to the rear of the machine to check the water level in the water supply tank The Operator to improve his view stood on the 5 inch Plexiglas containment berm and it collapsed causing him to fall He landed on his left shoulder and back sustaining a sprain and strain injury The Lead tried to gain access to the First [REDACTED] Room to contact OrthoLive for treatment advice but could not gain access The Operator was advised to sit and monitor the Wet Brush unit but after 10 minutes he notified the Lead that felt okay and was going home He proceeded to Urgent Care for treatment
