Before the incident
heavy pallets (200 lbs 72x48) were stacked flat like pancakes in the back of the warehouse. The top of the stack was approximately 5 ft high. Employee was removing the top-most pallet from the stack sliding it from the top of the stack to the floor.
What happened
Employee slid the pallet down to the floor then stood the pallet vertical. The pallet began to teeter and the employee attempted to jump out the way of the falling pallet.
Injury or illness
The pallet fell and struck the employee on the back of the foot heel and knocked the employee down to the ground in a push-up position.
