It is gather at the time of this report that an unknown foreign substance entered the employee 's eye sometime between the start of his work day and [REDACTED]. By [REDACTED] the employee stated that he discovered his eye was nearly swollen shut after asking another employee to take a look at it. At this point the employee informed his foreman and drove himself to Wills Eye ER.
