Before the incident
Performing a sortation machine diagnosis process on an abnormal sounding bearing.
What happened
During diagnostics the associate took the bottom plate guarding off in order to visually inspect the bearing and observe with a flashlight. Because the noise needed to be evaluated with the machine operating they needed to leave the machine on. The flashlight was small and did not provide sufficient light so the Associate brought the light closer to the bearing to see it better. Their hand was too close to the bearing and the tip of the flashlight touched the bottom of the moving belt which pulled their hand into the roller and belt. The Associate quickly pulled their hand out and sought immediate treatment.
