Redirecting a resident
What happened
Responded to call from the nurse in the neighborhood that resident was wandering around by the neighborhood exit door. Resident vocalized needing to use the bathroom. Took the resident by the hand to help steady him and help him walk to his bathroom. Resident then seemed to indicate that he wanted a hug. Allowed him to move in close as had not had any previous incidents with him. Unexpectedly he tried to grab me around the neck with his hand. Able to keep him from strangling me but he then grabbed ahold of scrub top with both hands. I tried to pry his fingers loose from my shirt but was unable strained neck back shoulders.
